Certified copies of birth and death certificates are available at the department for anyone who was born in or died in the state of Missouri. Out-of-state births and deaths must go through the respective state to obtain a certified copy. The Linn County Health Department’s clerical staff can assist you with this process.

It only takes a few minutes to fill out the application. Birth certificates are $15.00/copy and Death certificates are $14.00/copy.  If you are requesting multiple Death certificates, each additional certificate is $11.00/copy.

We only accept payment in the form of cash, check or money order.

We can only print Birth certificates from 1920 to present, and Death certificates from 1980 to present.  Birth or Death certificate requests before those dates will need to be sent directly to the State.

People who live out of state currently, or those unable to get to the office, can request copies be mailed to them. They need to fill out the form and mail to them at PO Box 280, Brookfield, MO, 64628 along with a check or money order.
